Recognition for previous experiences: credits can be granted, but no more than 50% of the duration of the apprenticeship. The credit experience can be granted by the sponsor of the program, but must be approved by the apprentice`s DAS signature: the apprentice must sign an agreement stipulating that they have read the terms of the agreement and that they agree with it. Once all the information has been completed, it must be signed and forwarded to DAS for approval before it can be used as an exported document. Start date: the date on which the apprentice must have signed the contract and received the DAS within 30 days of that date. Expected completion date: the date on which the apprentice completes the program and determines the status of a traveller (if a credit is granted for previous experiences, it must be taken into account when filling the completion date) Signature of the Union: if the sponsor of the program is part of a union, the union must also sign an agreement. The apprentice undertakes to do the work and take the corresponding courses. Recognize rights and obligations: Apprentice and sponsor must work where they are indicated. Agreements can be presented apprenticeship@mass.gov. Number of hours per day/week/overtime rate: the apprentice works one day, more than one week, and what is the rate of overtime for hours worked during the normal work week. Trade: the industry for which the apprentice files an apprenticeship contract is an obligation signed between the apprentice and the employer or sponsor. Previous employment rate: the level of salary received by the apprentice in a previous job.

School law: when the apprentice signs the apprenticeship contract, he accepts the conditions on the back of the agreement and gives access to all the corresponding teaching materials. Aggregate statistical information: The apprentice must provide DAS with his social security number, email address, date of birth, home phone number and check the corresponding boxes for gender, race, education and veteran status.
